Summary:
The Specialist III is responsible for all phases of program services including but not limited to participant development, and case management and rehabilitation services. This position will also use behavior intervention techniques to ensure that participants can be successful at achieving their goals.
Duties/Responsibilities:
- Oversee the supervision of participants enrolled in services.
- Support participants in achieving their goals while maintaining a safe and efficient work environment.
- Coordinate written and verbal input for evaluation reports and individual goal plans for participants.
- Conduct and document semi-annual and annual goal focused meetings with key stakeholders, scheduling meetings as needed Develop goals and report status via IPP.
- Liaison with family members, stakeholders, professionals, funding source representatives and other social services agencies.
- Coordinate activities designed to build skills and abilities and better position participants to achieve their individual goals.
- Provide written and verbal input for evaluation reports and individual goal plans for participants.
- Resolve participant issues as they arise (ex. Managing challenging behaviors).
- Effectively work with other Specialist III involved with CRP activities.
- Responsible for maintaining and updating participant database (CRIS).
- Coordinate or provide job coaching in the community as needed.
- Coordinate or provide transportation (drive) for participants to community jobs/volunteer positions/community outings as needed.
- Meet OSHA, Department of Labor and CARF requirements as needed. Comply with participant rights, Community Gatepath and program policies, CARF accreditation and quality assurance standards, Dept. of Labor, CAL-OSHA, etc.
- Provide flexible coverage as needed, including lunchtime support.
- Assist participants in all daily personal hygiene program: hand washing, toileting, including changing of clothing due to accidents.
- Support paid work and non-work activities as needed.
- Attend meetings as required.
- Other duties as assigned.
Physical Activity Requirements:
- Ability to consistently lift 50 pounds
- Able to lift, bend and kneel repetitively
- Ability to stand and walk for long hours
- Ability to respond to, lift and transfer participants with physical disabilities as needed
Education/Experience:
Any combination of education and experience that would likely provide the required knowledge, skills and ability is qualifying, unless otherwise specified. A typical way to qualify is:
- Bachelor’s or Advanced Degree in Social Service, Vocational Training, or related field, preferred.
Minimum of one (1) to two (2) years equivalent work experience working with the disabled population and the developmentall disabled population strongly preferred.
Licensing Requirements:
- Valid California Driver's License
- CPR Certification/First Aid Training
- Ability to obtain criminal clearance
- Ability to meet all licensing requirements.
Training Requirements:
- Required to complete a minimum of ten (10) hours of training or continuing education as directed and approved by the CEO and the CFO
