Summary:
The Specialist II, under the supervision of the Specialist III, is responsible for assisting in carrying out the daily activities of the program.
Essential Functions:
- Direct supervision of participants enrolled in services.
- Assist in creating and implementing program activities for participants.
- Assist in the development and implementation of individual program goals and writes progress evaluations at specified intervals.
- Assist in daily set up and clean up of program activities.
- Assess the program’s effectiveness in achieving participants’ IPP objectives.
- As required, assist in having a liaison with parents, care providers, funders and regional center case managers regarding participant progress and identifying problems as they arise.
- Oversee participants’ integration into the community adhering to program objectives while maintaining a safe environment.
- Assist participants in all daily personal hygiene program: handwashing, assist, if required, toileting, including changing of clothing due to accidents.
- Assist participants in daily lunch program.
- Assist in maintaining and updating participant database (CRIS).
- Assist in completing charting of each participant’s activities and assigned program required documentation.
- Conduct program activities.
- Comply with participant rights, Community Gatepath and program policies and procedures, and quality assurance standards.
- Must comply with applicable Community Gatepath Safety Policies, CARF accreditation standards, Community Care Licensing, CAL-OSHA, etc.
- Coordinate or provide transportation (drive) for participants to community jobs/volunteer positions/community outings as needed.
- Other duties as assigned.
Physical Activity Requirements:
- Ability to physically respond to physically frail persons - occasionally
- Ability to lift at least 50 lbs. - occasionally
- Ability to stand, stoop, and bend – frequently
- Ability to drive agency vehicles (transporting participants to events, etc.)
Education/Experience:
- Minimum Associate Degree is required.
- Experience in working with adults with developmental disabilities. College course work in social services, psychology or other pertenant course work, preferred.
- Positive outlook, enthusiasm, self-motivated and high energy.
- Ability to function as part of a team.
Licensing Requirements:
- Valid California Driver's License
- CPR Certification/First Aid Training
- Ability to obtain criminal clearance
- Ability to meet all licensing requirements
Training Requirements:
- Required to complete a minimum of ten (10) hours of training or continuing education as directed and approved by the CEO and the CFO
