Immediate Supervisor: Employment Services Coordinator

Summary:
The Job Coach is directly responsible for training and supervising all participants participating in the program at their work site in the community.
Essential Functions:
- Train and supervise participants on the job, on a weekly basis unless otherwise instructed
- Ensure participant work performance meets employer’s standards
- Maintain professional relationship with employer, and receive participant updates form the employer on a weekly basis
- Assist in the development of individualized training programs
- Maintain necessary records, evaluations and billing, and submit all paperwork for processing in a timely manner
- Maintain effective communication and acts as a liaison between the consumer, Department of Rehabilitation, and all other involved parties
- Ensure a safe continuous working environment according to general safety practices
- Participate in required meetings
- Report any participant or employer problems that might arise to the Supervisor or Department Manager
- Able to use personal vehicle to get to participant work sites in the community
- Must comply with applicable policies, i.e., Community Gatepath Safety Policies, CARF accreditation standards, CAL-OSHA, etc.
- Comply with participant rights, Community Gatepath program policies and procedures, and quality assurance standards
- Other duties as assigned.
Physical Activity Requirements:
- Ability to physically respond to physically frail persons - occasionally
- Ability to lift 50 lbs. - occasionally
- Able to work in a variety of work environments - frequently
Education/Experience:
- AA in Social Services, vocational guidance, or psychology (experience may be substituted for education) preferred
- One to two years’ direct-care experience with developmentally disabled population preferred
- Ability to work in different work environments (restaurant, maintenance, office)
- Able to work a flexible schedule including occasional week-ends, early and late hours
- Ability to work independently
- Professional attitude, and able
- Excellent problem solving, verbal and written communication skills
- Well organized
- Positive outlook, enthusiasm, self-motivated and high energy
- Ability to function as part of a team.
Licensing Requirements:
- Valid California Drivers' License
- CPR Certification/First Aid Training
- Ability to obtain criminal clearance
Training Requirements:
- Required to complete a minimum of ten (10) hours of training or continuing education as directed and approved by the CEO and the CFO
